Military Medical and Health Records
At some time in your life, you may run across a need for your military medical records. In my case, I was lucky enough to make a photocopy of my medical records before I had to turn them over to the Navy so they could lose them, which is exactly what happened.
When I started the VA disability process, I requested a copy of my records and I got 4 pages from my SRB, which were all illegible, and 6 pages from my medical record which were mostly blank, and two were torn in half. Well, hell, this is going to make the VA disability process that much more fun.
I was lucky as I had stated earlier to have a copy of my records. I digitized them all, so I can use them during the process as needed.
But if you were not lucky enough to grab a copy before you exited your military service, then fear not, there is a process that is supposed to help you obtain your records (the process that found 4 pages of my SRB and 6 pages of medical)
I would recommend that you start at the National Archives.
If that fails, then hit up the following:

U.S. Army Human Resources Command
All medical records regarding discharges that occurred after 1994 are located at the local Department of Veterans Affairs (DVA). Contact information for the DVA is located in the blue pages of the phone book. If by chance the constituent is unable to locate this contact information, they can call the DVA’s Congressional Liaison Service at (202) 225-2280 to obtain this information.
All medical records corresponding to discharges that occurred prior to 1994 are archived with the constituent’s Service Record Book at the National Personnel Records Center. They may contact this agency to request their medical records by writing to:

It is important to note that only one copy of medical records exists. Medical records cannot be ordered through separate agencies for this reason.
